Fulton County Judge Executive's office requires that open records requests be submitted using the appropriate form. The Attorney Generals office has produced a form and you can print the form and submit to our office. If you are applying for one and cannot print the form available on this website, you are welcome to make a written request, which shall include the applicants name, address, phone number, list of document he or she may be requesting, residency of the applicant, and anymore information that may help our office in fulfilling your open records request.
